I attest and verify that I have full knowledge of the risks involved in the event, that I assume those expenses and that I am physically fit and sufficiently trained to participate in the event. I release the rights to any and all photographic material personal information the Happy Tails and Trails Dog Walk/Festival may wish to release for the event without obligation to me.  I also acknowledge that my dog(s) in attendance have been properly vaccinated.
